NEW 2023-24 Achievements Program Manual - New Spark Achievement Program Information. First year program members begin with Spark Report. A completed record book will contain either a Personal Development Report (PDR) or a Spark Report. (not both)

Google Forms are now an acceptable option for Record Books. PLEASE READ before accessing Google Forms: 

Copy to your own personal Google Drive. NOTE: Most school districts block access from outside of your school district. Please be aware that if you are trying to use the google account assigned from your school, you are likely to get a denied access/request permission request. It is also likely that they will not grant you incoming access by your school account administrator. Please open a free, personal Google account to access the Google Record Book Forms feature.

All Google versions contain all forms, but does not include the My 4-H Story, Expression Pages, Collection of Work, or Resume for Seniors. There are prompts to remind you to create these pages.

All versions allow you to complete each form directly into the document and expand or delete the rows as needed. Use the forms style you are most comfortable using.

Record Book Forms for Primary Members:
Primary members can not earn star ranks for compete for county level awards; however, it's never too early to help members begin building record keeping skills! For those Primary members who wish to complete a record book, but are not quite ready for the regular record book forms, here is a from that can be used for recording information about Primary 4-H Years.
