2016/2017 Facility Use Form: Events


Alameda and Contra Costa 4-H Volunteers,

Please provide information requested below for your County Events.
The information will also be used to post the event to the county website and calendar.

The event survey is used for, but is not limited to, activities such as County Events & Field Days (presentation day, fashion review, food fiesta, showmanship clinics and more), Fundraising Events at the project, club or county level, Countywide Social Events, Countywide outreach.  If only your project or club is involved, or if your club or project is attending an event not hosted by your (eg: UCD Animal Science day, ARBA Rabbbit show), these are entered on the meeting survey.

Remember: Volunteers can not sign contracts and they need to be sent to the office.  They can be uploaded to the survey or emailed to the office.  

These are events that are supported and organized by Alameda or Contra Costa 4-H Volunteers.

Per 4-H Policy, “4-H members and adult volunteers are provided limited accident coverage when taking part in or attending an approved, regularly supervised 4-H activity”.

  • All facilities/locations for 4-H meetings (project & club), events & activities must be logged.
  • Please submit at least 30 days prior to the event/meeting so all paperwork (if required) can be processed.
  • Please read the following Clover Safe Notes regarding meeting spaces and/or your type of event
