Thank you for your interest in becoming a 4-H youth member or a 4-H adult volunteer. This survey is intended for 4-H program enrollment fees.

Enrollment Instructions

Payment is one step in 4-H enrollment. Please refer to the instructions below for the other steps. Your annual membership is good until September 15 of the program/school year.

Pay by check option: If you would like to pay by check instead, please make payable to UC Regents and mail to:
