ANR Recall to Active Service Checklist

This checklist includes all of the required forms such as: UCRP Reemployed Retiree Notification Form (UBEN 1039), State Oath Allegiance, Patent Agreement, Employment Eligibility Verification (Form I-9), UC Employee’s Federal-State Withholding Allowance Certificate (W-4 2019), etc.

Please note the following:

• State Oath Allegiance - If an employee has an active Emeritus appointment with ANR, this form is not required
• I-9 and E-Verify Tracker - If an employee has an active Emeritus appointment with ANR, this form is not required
• Non-Exempt Academic Appointees Compensatory Time Off (CTO) Election Form - required for hourly recalled retirees

All other forms are required unless stated as "voluntary".
