Facility Use Request Form

A Facility Use Request Form (FUR) is required for every CLUB/COUNTY EVENT, PROJECT or MEETING. This will allow the 4-H office to verify if the facility already has a Certificate of Insurance in place with UCCE. 

* Please allow a minimum of 10 to 14 days for processing of additional dates and times for existing insurance agreements. New agreements may take longer.

When do I need to complete the facilities use agreement?

  • Almost always, except when holding an occasional meeting in your house or at a restaurant. Some parks require a facilities use agreement.

How far in advance do I need to turn in the facilities use agreement form?

  • Processing may take 10-14 days minimum for existing agreements. New agreements may take longer. You can contact the 4-H office if you have any questions by calling (530) 889-7386.

What other information will I need to complete a FUR (Facility Use Request)?

  • You will need to contact the facility you wish to use, confirm and set a date, determine the cost and if they will require a certificate of insurance or a contract.

What do I do if the school or facility asks me to sign a contract?

  • Please do not sign any contracts. Only the County Director has the authority to sign contracts. If a contract is required, the contract is with the University of
  • California Cooperative Extension Placer Nevada (UCCE Placer Nevada).
