SHASTA MGP LIVE SCAN Take this form to any of the locations listed below to have your live scan (fingerprinting) performed. Volunteers are responsible for paying the fees. For "Level of Service", check only "DOJ" if you've lived in California for 2 years or more. Check both "DOJ" and "FBI" if you've lived in California less than 2 years.
Make sure you obtain a copy of the signed Live Scan form. After the live scan process is done, give a copy of the request form to Leimone Waite.
