HOW TO FILE A COMPLAINT
Discrimination/Harassment Complaint Form
The Discrimination Complaint Form can be used to file a complaint on the basis of race, color, national origin, religion, sex, gender, gender expression, gender identity, pregnancy, physical or mental disability, medical condition (cancer-related or genetic characteristics), genetic information (including family medical history), ancestry, marital status, age, sexual orientation, citizenship, status as a protected veteran or service in the uniformed services.
Use of the Discrimination/Harassment Complaint form is preferred but not required in order to submit a complaint.
Submit report online through:

Submit complaint via email to:

When submitting a report by email, we ask that you provide the following information: (1) the complaint type including the category of bias if applicable, (2) incident details, (3) name/contact information of alleged offender(s), (4) your name/contact information, and (5) attached any additional materials (e.g., photos, text messages, emails, receipts, etc.).
