Eat Healthy, Be Active (EHBA) Community Workshops Red Folders

These workshops are used for one-time classes. Although they can be used in a series format, if PSCC or ESBA is not being used.

ITC forms will be collected with each lessons. One side will have the intent to change questions (ITC), and the other side will have the demographic form below. As of 9/25/15 waiting for state office instruction for corresponding ITC questions. Demographics will be updated to remove the under 18 check boxes.
