What If...

...you have an Incident?

Incidents (accidents) happen, we know.  If you should have an incident, please be sure that you report it within 24 hours of the incident to be eligible to use the Hartford Insurance for reimbursement, and save your receipts.  Call the office at 408-282-3130 to make your report and send the forms below to UCCE staff. Disregard instructions to send anywhere else.  We will do that once we sign off on the incident form.

Please note that any incident resulting in property loss/damage and/or causalities (including property damage, theft, injuries to non-employees, and all auto accidents) must be reported using the Statement of Claim form
