Guidelines for Intervention Names
These naming guidelines are a starting point for naming Program Activity entries in PEARS. This document will assist your team in creating consistent and searchable program activity names.
This job aid will help you identify the appropriate site to use when reporting Indirect Activities, including when an activity is associated with multiple sites or not associated with a specific site at all.

Using Custom Questions for PSE Reporting
These job aids will help you with reporting organization-level and area-wide PSE efforts. For a video tutorial demonstrating how to use Custom Questions and Survey123, visit our training page.
First, you need to decide:
Is your PSE happening at the site-level, organization-level, or area-wide?
Site-level PSEs happen when PSE changes are implemented directly at the site.
This is the case for most PSE reports, and for these you do not need to answer additional questions in Survey123 about organization-level or area-wide efforts.
Site-level examples:
- an individual school
- store
- ECE site
- food pantry
Organization-level PSEs happen when the PSE change occurs at the “parent” organization that governs or provides direction for multiple sites. Click here for Organization-level PSE Job Aid.
You should provide information about the sites within the organization in Survey123. This means you no longer have to complete additional site-level reports for each site in the organization unless you are working with the site directly.
Organization-level examples:
- a school district
- a grocery association
- a food bank
Area-wide PSEs happen when PSE changes cover a geographic area of land and are not confined to a physical address. The LIA could partner with a site or an organization to implement area-wide PSEs. Click here for the Area-wide PSE job aid.
To describe the area where PSE changes are being made, you should complete additional questions in Survey123.
Area-wide PSE examples:
- developing or enacting city-wide policies
- capital improvements for a city’s sidewalk system that affect large parts of a city, rather than the occupants of a single building
