Site-Level Assessment Questionnaires(SLAQs) Project Overview
The SLAQs were developed in 2018-2019 and revised in 2019-2020 by the Nutrition Policy Institute (NPI) for use by CalFresh Healthy Living programs at California Department of Public Health (CDPH) and local health departments. SLAQs are designed to provide comprehensive annual site assessment data for program planning in four settings: schools, out-of-school, early care and education, and retail. NPI will also use data from the SLAQs about aggregated change over time, paired with information collected from the annual LHD Activity Checklists, for program evaluation purposes. Beginning in FFY 2020, California local health departments are expected to complete an annual assessment of all school, early care and education, out-of-school, and retail sites where they are engaged in site-level PSE work, using SLAQs.
