Facility Use Agreement & Certificate of Insurance Requests

Please complete the Facility Use Agreement and Certificate of Insurance Request form below for all organized meetings, activities, programs, and events.  4-H Adult Volunteers may not sign any contracts or agreements on behalf of UC 4-H. All requests must be processed by the UC/4-H Office.

Required Annual Year-End Reporting

Treasurer's Book with completed Financial Reporting Forms:

Form 6.2 - Annual Inventory Report (PDF)

Form 6.3 - Annual Financial Report (PDF): Amounts should match your bank statements.

Form 8.4 - 4-H Club Budget (PDF): Completed with estimated and actual totals for the completed program year, as well as a proposed budget for the upcoming program year.

Form 8.5 - Year-End Club Peer Review (PDF)

Form 8.6 - Year-End Club Peer Review Checklist PDF)

Form 8.7 - Fundraising Approval (PDF): Include any planned fundraising efforts noted on your proposed budget.

Club Constitution and Bylaws or Operating Procedures:

Required only if the 4-H Office does not have a copy of the most current version. Proposed changes should be approved through the 4-H Office during the program year.
