NPI has a variety of resources available to help with administration of assessment questionnaires.

The SLAQ questionnaires were developed for California Department of Public Health (CDPH) and local health departments by NPI to provide comprehensive annual site-level assessment data for program planning and evaluation in four settings: schools, out-of-school, early care and education, and retail.

In FFY 2025, the Elementary School SLAQ and Secondary School SLAQ are combined into a single School SLAQ. For more details, please check out the FFY2025 Summary of Changes on our Assessment Questionnaire Resources webpage.

LEAP Leaders in Eating and Activity Practices is an annual awards program developed by Nutrition Policy Institute to recognize sites annually for their achievements in meeting best practices for healthy eating and physical activity.

On September 13, 2022, we hosted a refresher training for all LHD staff conducting SLAQs in a learn setting (elementary school, secondary school, ECE or OST) in FFY 2023.
