A significant component of the 4-H Program involves service events and activities in our local community. It is common for a 4-H Club or project to host and complete numerous hours community service each year. It is important that this valuable work is recorded to share with our County Board of Supervisors, State 4-H Office, and County University Cooperative Extension (UCCE) Directors.
Please complete a survey upon completion of each service activity recording details such as number of participants, hours, locations, etc. The survey may be completed by adult volunteers, youth committee chairs, or community service club officers. Community Service Reporting Survey
