About the projects:
Impact Outcome Evaluation (IOE)
This evaluation project is required for all local health departments conducting a minimum intervention of series-based direct education in school or school-based after school programs with grades 4-12.

Assessment Questionnaires (AQs)
California local health departments work with school, early care and education, out-of-school time, and retail sites to complete Site-Level Assessment Questionnaires (SLAQs) each year where they are engaged in site-level Policy, Systems and Environmental (PSE) change work. A school district organization-level assessment questionnaire (OAQ) is available for LHDs working with school districts to implement PSE changes. Corresponding LHD Activity Checklists (LACs) provide information about CFHL interventions implemented

Adult Direct Education Evaluation
The California Department of Social Services requires all local implementers conducting adult direct education that consists of at least 4 sessions delivered over at least 4 weeks to participate in Adult Direct Education Evaluation.
