Alcohol Permit Request

When planning a UC ANR event where alcohol will be served, you must ensure the event is in compliance with UC policies and state laws concerning service of alcohol. This UC ANR alcohol permit request form will verify compliance with risk management standards, UC policy that prohibits purchase of alcohol with certain fund types, and state laws concerning licenses to serve alcohol.

Request for Alcohol Permit on ANR Workflow Automation Application

Effective 8/2/2021, this is the preferred way to submit a request for an UC ANR Alcohol Permit. The new request for Alcohol Permit application is part of a suite of workflow automation tools that are being deployed by UC ANR to provide increased efficiency, transparency, and security for commonly used forms. Use the link above to access the Request for Alcohol Permit form.

To access the request for Certificate of Insurance form, go to the ANR Workflow Automation Application. Once you are logged in, click on "Risk & Safety Services" on the left-hand menu, then click on "Alcohol Permit" to start a new request.
