Certificates of Insurance
When entering into agreements with other entities, UC ANR is often required to provide proof of insurance or a Certificate of Insurance. The information below provides some options about how to fulfill this requirement.
Generic Certificate of Self-Insurance
This generic Certificate of Self-Insurance Coverage may satisfy many vendors or agencies who are requesting proof of insurance from the University. We recommend that when a certificate of insurance is requested by a business or organization, that you first provide the above generic Certificate of Self-Insurance. If this generic certificate does not meet the requestor's needs, then proceed to the options below for requesting a Certificate of Insurance.
Request for Certificate of Insurance on ANR Workflow Automation Application
Effective 8/2/2021, this is the preferred way to submit requests for Certificates of Insurance (COI). The new request for COI application is part of a suite of workflow automation tools that are being deployed by UC ANR to provide increased efficiency, transparency, and security for commonly used forms. Use the link above to access the Request for Certificate of Insurance form.
To access the request for Certificate of Insurance form, go to the ANR Workflow Automation Application. Once you are logged in, click on "Risk & Safety Services" on the left-hand menu, then click on "Request for Certificate of Insurance" to start a new request form.

If you have difficulty accessing the Request for COI form, or the new Workflow Automation Application, please contact the dedicated support email support@ucanr.edu.
If you cannot access the COI request on the Workflow Automation application, then you may use the Attachment C: Request for Certificate of Insurance. Complete and submit the form to olharris@ucanr.edu
