How to Become a Cooperator
(to report a tree that has failed in California, and soon, Hawaii, Nevada and Arizona)
It's easy. We are encouraging the use of the International Tree Failure Database (ITFD) report forms, so we ask you to:
- download and read the ITFD User Guide,
- download and print the ITFD form, fill it in and
- mail it to us at:
- WTFD/ CTFRP, P.O.Box 725, Larkspur, CA 94977-0725
- or, e-mail to treefail@mac.com
- photos included with reports are appreciated
Some good news! Thanks to Igor Lacan (UCCE Urban Forestry Advisor) we now have an interim solution to make it easier to create failure reports...no paper involved.
Here's what you do:
1.On his website, he has made three icons that, when clicked, lead straight into the “branch failure,” “trunk failure” and “root failure” surveys, respectively. The page is here:https://ucanr.edu/sites/Igor/Western_Chapter_ISA___ITFD_Tree_Failure_Form_On-line/
2. Click on the icon for whichever type of failure it is.
3. Fill in the form as usual. When you get to the end it will prompt you to save and that's it. If you have more than one failure of the same type, leave the site and start again.
4. He also made a short video, explaining HOW to add that webpage as a shortcut to a smart phone’s home screen, so it looks just like an app.

5. It is now possible to submit photos on this site.
6.. You don't have to do it this way. Whatever way is most convenient for you is fine.
