Documents
-
Club Information Form (Required)
This is critical information that 4-H Staff need for your unit. We enter this information online in ZSuites at the beginning of each year. Without accurate information, families will not have the correct projects to enroll in and we may not be able to contact key volunteers in your club. -
Annual Club Planner (Required)
This is the document to use to plan your program year. What community service, events or activities are you doing? -
4-H Program Planning Guide
This document guides you on the necessary requirements to maintain a chart/club. -
Outreach Documentation Form (Required)
Did you know? This U.S. Department of Agriculture (USDA) required document. But it is about more than increasing enrollment. It is about showcasing your club as a part of the community and bringing a positive outlook and education to 4-H. -
4-H Constitution Template
Constitution: This is the foundational document. It outlines the basic structure and purpose of the club—things like the club's name, mission, membership, and what officers exist. In short, what the club is. -
4-H Bylaws Template
Bylaws: These are the detailed operating rules that explain how the club functions day-to-day. They cover things like how officers are elected, how meetings are run, how votes are taken, and how committees are formed. Bylaws are more flexible and can be changed more easily to fit the club’s needs. In short, how the club works. -
